Documents required for admission:

1. Admission Form duly filled and signed by student and Parent/Guardian (Front and back page both)
2. 2 PP size photographs of student
3. 2 photocopies of Student CNIC/B Form. Please also bring Original CNIC/B Form for verification
4. 2 photocopies of Parent/Guardian CNIC. Please also bring Original CNIC for verification
5. 2 photocopies of Matric Degree & Detailed Marks Certificate. Please also bring Original degree for verification
6. 2 photocopies of F.Sc Pre-Medical Degree & Detailed Marks Certificate. Please also bring Original degree for verification (Only for DPT admissions).
